Compensation: $35.73–$39.27/hour
Schedule: Monday–Friday, 8:00 AM–4:30 PM (35 hours/week, no weekend rotation)

Position Summary

Provide outpatient rehabilitation services primarily in an in-home setting for the older adult population. This includes patient care in private residences or within independent living facilities enabling therapy in the most functional environment: the patient’s home.

This role offers a supportive work environment in a “Great Place to Work” certified community for 2023 & 2024, with access to an experienced rehab team and electronic documentation via Net Health using individual iPads.

Responsibilities

  1. Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel as required.
  2. Adhere to treatment schedules, documentation standards, and departmental requirements.
  3. Attend and contribute to patient care and team meetings.
  4. Collaborate with rehabilitation teams and community physicians
  5. Develop individualized therapy plans addressing areas such as:
    • Cognitive skills/ Adaptive devices, orthotics, and prosthetics
    • Muscle strength and coordination/Endurance and mobility
    • Balance and sensory awareness/Activities of daily living (ADLs)
    • Fall risk management and medication management
  6. Instruct and train patients in functional activities and use of adaptive devices.
  7. Monitor and document patient progress, reevaluating and updating treatment plans
  8. Provide case management and community integration support for patients.

Requirements

  • Valid Occupational Therapy license in Michigan or proof of eligibility.
  • A minimum of 1–2 years of experience (new graduates not eligible).
  • Must possess a driver’s license or reliable transportation.
  • Strong organizational, communication, and time management skills.
