Full-time Community Care Manager – Social Worker for the Maryland Primary Care Program (MDPCP).
Hybrid Role- Two days a week in the Annapolis Office.
Majority care coordination and patient advocacy role, ensuring high-quality patient support and access to community resources.
Full-Time – Day Shift: Monday – Friday (No weekends)
Pay: $35.00 – $50.00 per hour
Expected Hours: 40 per week

Essential Job Duties:

- Identify social determinants of health and connect patients to community resources.
- Serve as a patient advocate, care navigator, and liaison between providers and support services.
- Lead and support the care team in improving care coordination and managing referrals.
- Facilitate health education to empower patients in self-managing chronic diseases.
- Collaborate with patients, care teams, and providers to create and maintain a longitudinal care plan.
- Coordinate post-hospital and ED discharge transitions for high-risk patients.
- Screen and identify patients at risk of avoidable hospitalizations or readmissions.
- Participate in data collection, health outcomes reporting, and quality improvement initiatives.
- Evaluate care plans and resource utilization to improve patient support programs.
- Contribute to team performance evaluations and continuous improvement efforts.
Educational/Experience Requirements:
- Master’s degree in Social Work (MSW).
- LCSW-C license in Maryland (or ability to obtain within six months).
- At least five years of experience in healthcare , including care management.
- Knowledge of behavioral health, chronic disease management, and community resources.
- Proficiency in Epic, CRISP, and patient registries preferred.
- BLS CPR certification required.
- Case Management Certification strongly preferred.
Required License/Certifications:
- LCSW-C from the Maryland Board of Social Work Examiners (or LCSW with ability to obtain LCSW-C within six months).
- BLS for Healthcare Providers certification (American Heart Association).
